A medium apple carries about 19 grams of sugar. So does half a can of cola. Under a microscope the molecules are essentially the same — glucose and fructose in similar ratios. Yet one is a food you can eat every day without thinking, and the other belongs to the category that most reliably pushes people past their sugar budget. The grams match; almost nothing else does.
The difference is packaging. The apple's sugar is locked inside intact plant cells, bundled with roughly four grams of fiber and a lot of water. You have to chew it, your gut has to dismantle it, and the sugar reaches your blood as a slow drip. The cola's sugar is already dissolved. There is nothing to chew, nothing to slow it down, and no fiber to tell your appetite that anything happened. Same molecule, completely different delivery.
That distinction — added versus naturally occurring — is the backbone of modern sugar guidance. It explains the roughly 50-gram daily cap, the "Added Sugars" line on the nutrition label, and why a grading algorithm can wave an apple through while marking down a fruit drink with identical sugar numbers.
Where the 50-gram cap comes from
The World Health Organization recommends keeping "free sugars" under 10 percent of daily calories. On a 2,000-calorie diet, that works out to about 50 grams — roughly 12 teaspoons. The FDA landed on the same number: the Daily Value for added sugars on US labels is 50 grams, which is what the percentage next to "Added Sugars" is calculated against. The WHO goes further, suggesting that dropping below 5 percent — about 25 grams — likely brings additional benefit.
The key phrase is free sugars. That means sugar added by a manufacturer or a cook, plus sugar naturally present in honey, syrups, and fruit juice. It does not include the sugar inside intact fruit, vegetables, or plain milk. Nobody has built a serious case against eating whole apples; the evidence pointing at sugar — weight gain, dental decay, metabolic strain — comes overwhelmingly from sugar that has been extracted, refined, and concentrated into foods and drinks.
Two consequences are worth internalizing. First, the cap is not a total-sugar cap: the lactose in plain yogurt and the fructose in a peach don't count against it. Second, juice does count — squeezing fruit frees its sugar from the structure that made it slow.
Sugar goes by more than twenty names
Ingredient lists are ordered by weight, and manufacturers know it. Split the sweetening across three different sugars and each one lands further down the list, making the product look less sugary than it is. That is why it pays to recognize the aliases:
- The obvious ones: sugar, cane sugar, brown sugar, beet sugar, invert sugar, coconut sugar
- The syrups: corn syrup, high-fructose corn syrup, glucose syrup, rice syrup, maple syrup, agave, honey, molasses
- The chemistry names: dextrose, fructose, sucrose, maltose
- The stealth entries: fruit juice concentrate, malt extract, and maltodextrin — the last is not technically a sugar and barely tastes sweet, but its glycemic index is actually higher than table sugar's

Drinks are where the budget actually breaks
Sugar-sweetened beverages are the largest single source of added sugar in the typical American diet — bigger than desserts, bigger than candy. The math shows why a single drink can consume most of a day's cap:

Liquid sugar is uniquely bad at the one thing food is supposed to do: register. Drink 300 calories of soda and your appetite barely adjusts — you eat the same lunch anyway. There is no chewing, no fiber, no bulk, so the signals your body uses to count food never fire. That compensation failure is the core argument in our piece on liquid calories.
Note the juice row. A glass of 100% orange juice contains no added sugar at all — and it still counts as free sugar, because pressing the fruit did the work a spoon would have done.
"Natural" is not a hall pass
Honey, maple syrup, agave, and coconut sugar are natural in origin and free sugars in effect. Your metabolism does not check the marketing. All of them count against the cap, gram for gram.
Two edge cases deserve nuance. Dried fruit keeps its fiber, which genuinely separates it from candy — but drying concentrates sugar dramatically, so a small handful delivers the sugar of several servings of fresh fruit. And smoothies sit in between: blending a whole apple keeps the fiber but shreds the cell structure that made the sugar slow, so blended fruit behaves more like juice than like fruit. How Nutrication uses this
Nutrication's six-measure grade treats added and natural sugar differently at several points in the pipeline — deliberately.
The natural-sugar exemption. Inside the Nutri-Score measure, worth up to 30 points of the grade, sugar is normally penalized in ten steps starting at 4.5 g per 100 g. But if a food is whole or minimally processed, has no added-sugar ingredient on its label, and is not a beverage, the engine counts its sugar penalty as zero. An apple's sugar takes none of the ten possible penalty points; the score breakdown displays it as "natural — not counted," and the app surfaces the highlight "Sugar is naturally occurring (not added)." Apples, plain yogurt, milk, and dried fruit are the intended beneficiaries.
Drinks never get that mercy. Beverages are scored on much stricter thresholds — the first sugar threshold sits at zero grams, so any sugar in a drink immediately costs points. Only plain water can earn the top beverage rating, and in practice a sugared drink lands at a C or worse on that measure. The engine's stated reasoning: liquid sugar, even blended whole fruit in a smoothie or fresh juice, spikes blood glucose without the whole-fruit matrix.
Detection runs on the alias list. To decide whether sugar is added, the engine scans ingredients for more than twenty markers — the same names listed above, from cane sugar to fruit juice concentrate to maltodextrin. One match and the exemption is off.
Ingredient flags weigh in. High-fructose corn syrup costs 5 of the ingredient-quality measure's 20 points, citing the American Heart Association. A credible "no added sugar" or "unsweetened" claim earns 3 points back.
Glycemic load seals it. A soda is estimated at a glycemic index of 70 and — when the label doesn't state a serving — assessed on a full 330 ml can, the engine's default for canned drinks, not a token 100 ml pour. Whole fruit like an apple or a peach is estimated at 40 (higher-sugar fruits like bananas and grapes at 55); juices and smoothies at 55 — the same rank order the biology suggests.
What to actually do
- Budget roughly 50 g of added sugar a day — 25 g if you want the stricter target — and spend it consciously.
- Get your sugar from foods you chew. Whole fruit is effectively exempt; drinks are where budgets quietly die.
- Read the "Added Sugars" line, not "Total Sugars" — plain yogurt looks sugary on totals alone.
- Scan ingredient lists for syrups, "-ose" endings, and juice concentrates; three aliases usually means one big dose split up.
- Treat juice and smoothies as occasional small servings, not as fruit equivalents.
